Appointments
Appointments are available Mon/Thursday/Friday and Saturday at our convenient Northgate location at 2111 N Northgate Way, Ste 217, Seattle Wa. 98133. We are located just west of I 5 and the Northgate Mall. Click here for directions to the office.
First consults are 75 minutes, follow up appointments are 50 or 30 minutes. Prior to your first appointment, download and fill out the following forms:
- Appointment form
- Symptom questionnaire
- Food Diary (keep seven day food diary with symptoms if necessary)
Bring these forms to the first appointment. Also, bring a list of your medications, and bring all your nutritional supplements, in their bottles to the appointment.
